Polyene Macrolide

Dosage, Route, and Frequency

Amphotericin B (0.1 mg/mL in 5% dextrose)

0.25–1 mg/kg, IV slowly, 3 times/week

Total dose: 4–11 mg/kg or until azotemia occurs (cats, dogs)

1.5 mg/kg, IV, q 8 h for 3–5 days (birds)

1 mg/kg, nebulized over 15 minutes, q 24 h for 10–14 days (birds)

0.3 mg/kg, IV, q 24 h for 3 days (horses)

Amphotericin B lipid complex injection (Abelcet®)

1–3 mg/kg, IV, every other day or 3 times/week

Total dose: 12–24 mg/kg (dogs, cats)

Amphotericin B liposome (AmBisome®)

3– 4 mg/kg, IV, every other day or 3 times/week

Total dose: 12–30 mg/kg (dogs, cats)

Nystatin

50,000–150,000 U, PO, q 8 h (dogs)

100,000–300,000 U/kg, PO, 8–12 h for 7–10 days (birds)

Pimaricin (5% ophthalmic solution)

1 drop of 5% suspension, q 1–2 hours initially, then decreasing to 6–8 times daily
